Benign Prostatic Hyperplasia and Its Aetiologies

چکیده
Context: Benign prostatic hyperplasia (BPH) is a well-known condition characterised by prostate growth accompanied by lower urinary tract symptoms. Several mechanisms seem to be involved in the development and progression of BPH. Objective: To review the most important findings regarding the key mechanisms involved in the pathophysiology of BPH. Evidence acquisition: During the 2009 annualmeeting of the European Association of Urology in Stockholm, Sweden, a satellite symposium was held on BPH and its treatment. This paper is based on one of the presentations at the symposium. A structured, comprehensive literature review was performed using data retrieved from recent review articles, original articles, and abstracts. Evidence synthesis: Several mechanisms seem to be implicated in the pathophysiology of BPH. These represent age-related tissue modifications, hormonal alterations, andmetabolic syndrome as well as inflammation. Although androgens do not cause BPH, the development of BPH requires the presence of androgens. Moreover, several studies support the association between noninsulin-dependent diabetes mellitus, hypertension, obesity, and low high-density lipoprotein cholesterol and the development of BPH. Finally, recent increasing evidence seems to support the idea that BPH consists of an inflammatory-based disorder. Inflammation would be initiated by an unknown stimulus that would create a proinflammatory milieu within the gland. This theory is confirmed by several basic research and clinical studies that showed a statistically significant association between inflammation and BPH severity and progression. Conclusions: Although the pathogenesis of BPH is not yet fully understood, several mechanisms seem to be involved in the development and progression of the disease. Thesemainly include systemic and local hormonal and vascular alterations aswell as prostatic inflammation that would stimulate cellular proliferation. Inflammation would be initiated by an unknown stimulus that would create a proinflammatory environment within the prostate. Therefore, from the recent clinical and basic research studies, a novel approach in the clinical management of BPH might focus on the inflammatory process involved in the development and progression of the disease. # 2009 Published by Elsevier B.V. on behalf of European Association of Urology.

Evaluation of c-Myc mRNA Expression Level in Benign Prostatic Hyperplasia and Prostatic Adenocarcinoma Tissues and Its Correlation with Clinicopathological Characteristics
Background and Aims: Prostate cancer (PCa) is one of the most common cancers among men in Iran. Since changes in the regulation of proto-oncogenes expression are the main causes of most human cancers, including PCa, evaluating the expression of marker genes can be helpful for early diagnosis of cancer and better understanding of its etiology. The present study compared c-Myc expression level in...
متن کاملDIFFEREN TIATION BETWEEN PROSTATIC\' C A RCINOMA A ND BENIGN PROSTATIC HYPERPLASIA BY AgNOR STAINING
Thirty cases of benign prostatic hyperplasia and 30 cases of prostatic adenocarcinoma were selected and stained by AgNOR (argyrophilic staining of the nucleolar organizer region) method. Specimens were obtained by either prostatectomy (open!TIJR) or needle biopsy of the prostate. In this study, the sections of prostatic adenocarcinoma were reviewed. 6 cases were diagnosed as well-differen...
متن کاملTHE INCI DENC E OF PROS TATIC INTRAEPI THELIAL NEOPLASIA IN PROSTATIC HYPERPLASIA AND PROS TATIC C ARCINOMA
Prostatic intra-epithelial neoplasia (PIN) is considered a precursor of invasive carcinoma, characterized by proliferation and anaplasia of cells lining prostatic ducts and acini. In this study, we applied the PIN grading system on one-hundred cases of prostatic carcinoma and one-hundred cases of benign prostatic hyperplasia, then compared the results. 74% of prostatic carcinoma and 51 % of...

benefits due to its very potent anti-inflammatory and antioxidant properties. Despite its widespread use, it has not been validated for use in the treatment of benign prostatic hyperplasia (BPH). This study was conducted to examine histomorphological effects of SJ seed on BPH that usually causes bladder outlet obstruction. Materials and Methods: There were a total of six groups of animals each...
متن کاملAntioxidative effect of hydroalcoholic extracts of fruit and pit of Elaeagnud angosifolia against oxidative stress markers in benign prostatic hyperplasia
Aims: The use of medicinal plants has been common among people since ancient times, and in recent years special attention has been paid to their therapeutic use. Since oxidative stress is one of the probable causes of benign prostatic hyperplasia, the present study aims to compare the effect of hydroalcoholic extract of fruit and pit of Elaeagnud angosifolia on oxidative stress factors in pati...
متن کاملNon – Detection of HPV DNA in Prostatic Cancer and Benign Prostatic Hyperplasia: a case- control study in Kerman
Background: Different studies assessed the role of inflammation in malignancy of different organs. Prostatitis has been suggested as a cause of cancer and BPH. Till now, more than 100 types of human Papilloma virus (HPV) are recognized including low and high-risk groups for carcinogenesis. Among them HPV-16 and HPV-18 have shown further association with cancer. Regarding the pr...
